I notice that occasionally the output of an image stack shows up with a small red triangle icon - and I could not find this mentioned in the documentation. I assume it means there was some problem(s) with the focus stacking, but not sure what.

You're right, it means there was a minor problem with your stack which might negatively impact the quality of the result. Hover the mouse over this item to see the detailed message.
